Diagnosis is difficult to perform and difficult to teach as it requires a large volume of information of various kinds and an appreciation of the differential diagnosis process. The fourth version of the Refiner series, AKT-R4, incorporates a new concept known as the diagnostic web, which is a combination of hypothetico-deductive reasoning (HDR) and illness scripts. AKT-R4 requires no additional knowledge acquisition beyond the loading of a set of cases which the system uses to build a model of the domain (although contextual factors can be added to provide a finer degree of control over the diagnostic process). The system will be evaluated in terms of accuracy of diagnosis, speed of diagnosis and ease of use.
